To save a little time you can have your application already filled out before you come into the office. Click on the appropriate link below, print out the application form, fill it out completely (all information is required) and bring it with you when you come into the office to sign up for service. Don't forget your 2 forms of identification and $12.00 application fee as stated in the application procedures above. Renters will also need to remember their signed lease and $20.00 deposit.

If you are out-of state, purchasing a home in Lafayette and have never had water service with us,you may fax your completed application form to 765-807-1153. You will be billed for the $12.00 application fee and will be required to come in at a future date to present your 2 forms of identification.
